TriCounty Health Council Formed


2016

Collaborative initiatives began:

  • Readmissions Reduction for Congestive Heart Failure, Chronic Obstructive Pulmonary Disease (COPD), and Pneumonia

  • Primary Care, Behavioral Health Care Integration Project

  • Opioid Medication-Assisted Treatment


2017

Added the following initiatives:

  • Integration of Community OB Services

  • Primary Care - Behavioral Health Care Integration Project

  • Collaboration between TCHC Members and CBOs

  • Securing Revenue Streams


2018

Formed work groups to progress with:

  • Opioid Epidemic

  • Community OB Services integration

  • Primary Care and Behavioral Health Care Integration

  • Communications


2019

Work Groups focused on specific initiatives:

  • Pottstown Hospital-Tower Health and Pottstown Area Health & Wellness Foundation completed Community Needs Assessments to inform work group direction in 2020

  • Community OB Services Integration Work Group completed its work in 2019 when the transition of the OB clinic from PH-TH to CHDC was completed


2020

January:

  • Conducted Priority Survey to set direction of 2020 Work Groups

April:

  • Participant Organizations joined to collaborate around the community’s COVID-19 Response

June:

  • Kicked off TCHC Campaign to Save Lives

September:

  • Primary Care and Behavioral Health Integration Work Group completed its work with the conclusion of the SAMHSA grant

October:

  • Work Groups resumed efforts by confirming membership and drafting aim statements

  • Campaign to Save Lives: Phase I - Safely Get the Care You Need - provided multi-media material to share electronically, printed and distributed postcards, and issued press release that was picked up and published by the Pottstown Mercury

December:

  • Campaign to Save Lives: Phase II - Don’t Delay Your Healthcare - Completed in December

  • TCHC Brochure Finalized

2021

January:

  • First TCHC Town Hall held on January 12th, “The Importance of Vaccines - Building Trust within Communities of Color”

  • Refined Board and Governance Structure of TCHC

  • TCN became a partner organization

February:

  • Kicked off TCHC Smile Campaign, which included the first use of TCHC billboard advertisements to promote community initiatives
